ELAS surgery performed in Super speciality Hospital Jammu

Endovascular Leaking Aortic aneurysm surgery performed in Department of Cardiovascular & Thoracic Surgery Super speciality Hospital Jammu.

68 years old male who presented with cough and pain in the chest radiating to back visited Cardiovascular & Thoracic Surgery Govt Super speciality hospital Jammu.

He was investigated and found to have large defect in the largest blood vessel (Aorta) carrying blood from heart to the whole of body, because of weakness in its wall and was leaking into the surrounding areas in chest and left Lung. This leaking Aortic Aneurysm has led to blood loss and his Haemoglobin had dropped to 7gm. On top of it same patient had malignancy in the right Lung. Patient had taken consultation outside the state in Punjab In corporate hospital and also at PGI Chandigarh, and finally landed with Cardiovascular & Thoracic Surgery at Government Super speciality hospital as he could not afford Endovascular Grafting at these centres.

Patient was very high risk because of his associated other ailments.

“Such patients pose challenges both for Cardiovascular & Thoracic Anaesthesiologists and Cardiovascular & Thoracic Surgeons and need to be planned meticulously as morbidity and mortality can be high and endovascular procedures have to be backed up for open Surgery and costs of endovascular procedure disposables are high”  Dr Shyam Singh Head of Cardiovascular & Thoracic Surgery and Professor Dr I.A Mir explained.

Dr Puja Vimesh, Professor & Head Department of Cardiovascular & Thoracic Anaesthesiology said “A fine balance of blood controlling drugs to maintain adequate blood pressures during endovascular procedure is key to outcome of such Critical Patients”

Patient underwent deployment of Captiva Valiant Endovasular Aortic graft through the right groin artery(femoral artery) to seal the leaking defect and cover the proximal and distal areas of aorta on 03.12.2025. All the expenses were covered under Ayushman Bharat Scheme.

Cardiovascular & Thoracic Anaesthesia team headed by Professor Dr. Puja Vimesh consisted of Dr Vikas Gupta, Dr ReenaVerma, Dr Santosh Gupta, Dr Sonamdeep, Dr Tarun assisted by Mr Vikas Sharma Seniormost technologist, Mr Saleem, Mr Arif, Mr Kazim, Mr Karan, Mr Sheikh Rouf, Mr Tarun.

The operative team consisted of Dr Shyam Singh, Dr I.A Mir, Dr Mohit Arora, Dr VivekGandotra, Dr Rouf Gul, Dr Prabhdeep Singh, Dr Anirudh (DrDNB CVTS Trainee) assisted by Senior Perfusionists Mr Charanjit Singh & Mr Roshan Lal (incharge O. T) ,ably helped by Mr. Waryam Singh,  Senior O.T staffs Vishali (supervisor CVTS) BalvirKour, Sonia Sharma, RenuNathyal. Others who helped in procedure were Frena, Arjumand, Zahida, Rukya, Sajjad, Deepak, Shubham, Dilaer Singh C-arm technician.

This procedure was possible only because of the help from Dr Haroon Salaria Professor & Head Department of Neurosurgery and Neurosurgery OT staff led by incharge Annie Kokhar for providing C-arm, Neurosurgery OT.

In Cardiovascular & Thoracic Intensive Care unit SSH, patient was monitored by highly trained Staff sisters and Cardiac anaesthesia staff which included Sisters – TajinderKour(incharge ICU, SSH), Sayeeda, Nayeem, Vandana, Shivani, Shivalika, Ekta, Nighat, Sonali, Sonakshi, Pooj Urmila, Rajinder, Neetu and Mr Ashish.

Principal Professor Dr Ashutosh Gupta congratulated the teams involved and appreciated dedicated efforts of Department of Cardiovascular & Thoracic Surgery and Department of Cardiovascular & Thoracic Anaesthesiology and Medical Superintendent SSH Dr Rehana Khurshid and Dr Sanjogita Soodan for providing logistic support.
